Initial Adult Visit (up to 90 minutes) - $210 --> $250
Initial Pediatric Visit (up to 75 minutes) - $170 --> $190
1 Hour Follow-up - $150 --> $170
45 Minute Follow-up - $110 --> $130
30 Minute Follow-up - $80 --> $100
15 Minute Follow-up - $55 --> $75

Direct billing is available only during office hours at Spring Forward Health, depending on who your insurer is.
